Individual Consultations
 

Initial consultations include 10 minutes of paperwork, followed by discussion related to your personal goals. A medical history as well as a symptom survey will provide information so that recommendations for dietary change and supplements can be made. Follow up visits will involve "fine tuning" of the program, evaluation of results, and on-going support.


Appointments are available for individual consultations

Monday through Friday
9:30a.m. to 6 p.m.,
and evening hours as needed.

Telephone consults can also be arranged.
 

The charges for individual consults are

$100.00 for a 90 minute visit,  
$75.00 for a 60 minute visit,
$40.00 for a 30 minute visit.

Payment will be expected in full at time of visit.
 

Regarding insurance, Shoshana will supply you with a master bill, which will include a procedure code and diagnostic code that can be submitted to your insurance company for reimbursement.
